What You Will Learn
Claim Submission
→ Claim is submitted electronically (Clearinghouse) or directly through the Provider Web Portal.
Claims Processing (Payer Side)
→ Insurance reviews the claim and updates the status in the Provider Web Portal.
Claim Status Check
→ Billing team checks claim status through the Provider Web Portal (Approved / Denied / Pending).
Denial / Partial Payment
→ If denied or underpaid, denial reason codes are viewed in the portal (EOB / ERA).
Appeal Submission
→ Appeal can be submitted via:
Provider Web Portal upload
Fax
(Portal submission is faster and trackable.)
Reprocess
→ After appeal approval, the payer reprocesses the claim. Updated status is reflected in the portal.
Resubmit (If Rejected)
→ If the claim is rejected for errors (invalid code, missing modifier, etc.), correct and resubmit via clearinghouse or portal.
Write-Off
→ Contractual adjustment posted after payment confirmation (checked in portal).
Waiver of Liability
→ If the payer confirms the patient is not responsible, documentation is verified through portal notes.
